Most of us (especially, Indians) form a sentence in our native language, translate it in English and then use it in a conversation. You have to stop doing that. Start thinking in English. Its a bit tough, but it will become easier and spontaneous with time. Don't think in your native language, think in English. This is the biggest hurdle in the process of improving your communication skills. Once you cross it, nothing can stop you.
Now how to do that? Simple. Speak with your mind whenever you get time. But speak in English. Don't worry about grammar. Just speak.
Now, let's focus on improving your fluency.
Start reading newspapers, magazines, novels, blogs and whatever you can. But don’t just read, try to understand it. And make an opinion about it. Especially about editorials, opinions and issues.

One more activity that might help. I assume you have a laptop or pc (even a smartphone would work). It will not only improve your pronunciation but also boost your confidence.
One more activity that might improve your fluency in English. Find a friend/relative/anyone (being an Indian, I know that this can be a bit tough as whenever you start speaking in english, you are accused of "shininig") who speaks English fluently and converse with him as much as possible. Ask him/her to point out your mistakes and tell you how to correct them

8. Get the basic knowledge of adverb adjective noun verb and with one word you would get 4 way to use it. This will give you an idea how to change the word according to sentence. If you know how to use it in different sentences then it will give you an edge over others. If you have this elementary things along with you can explore it anywhere.

Actually this one is from my elder brother long time back I was learning typing but I was stuck at the speed of 20 word per minute and no matter how much I tried I was unable to increase my speed then one fine my brother came to me and advised to do typing until your fingers get paining and you won’t believe I started improving. I am assuming you know the basics, since you asked this question in English and are ready to understand the answers in English. May be just need knowledge on proper use of grammar, fluency and confidence to use it in your work?
I would suggest you to read English newspapers/magazines/novels daily, preferably read out, while trying to understand its meaning and being conscious about how the sentences and grammar are used.
And start talking in English, however broken it is, at least with your closed ones. Make a rule to talk in only English for few hours daily, and may be reward yourself for following it.
If you watch TV series or movies then watch them in English. Use subtitles and try to get the context. You may have to pause in between to understand better.
Also keep an English dictionary to look up the meaning of the words.
